Select one of the aliases presented in the drop down list and click ‘Submit Alias’. 

If you do not wish to create an Alias at this time, please click ‘Cancel’ 

If there are no valid options press ‘Back’ to be returned to the previous screen where you can enter in alternate / new information into the First, Middle, Last Name and Number, and create additional alias names to choose from.
