Your alias name can contain the following information:
First Name: This field has been pre-populated, however you can edit this field to contain which ever first name you want. Or you can leave this field blank.
Middle Name: You can enter any name you want into this field, or leave this field blank.
Last Name: This field has been pre-populated, however you can edit this field to contain whichever last name you want. You must fill this field in, it is a required field.
Number: You can enter any number you wish into this field. You can leave this field blank if you wish.
Once you have filled in the fields as you wish please select ‘Next’ to view the alias options that have been created based on the above information.
Click ‘Back’ to go back to the previous Screen
If you do not wish to create an Alias at this time, please click ‘Cancel’
